WHAT WE DO

  • One of our trained professionals will assess the environmental, cognitive, and general well-being of each individual and provide recommendations for living life to the fullest in their preferred location.

    We also offer Emergency Assessments when unexpected falls or illness occurs.

  • Our professionals are also available to act as a liaison between family members, caregivers, and medical professionals to ensure your voice is heard and your final wishes are respected.

  • We’ll be a call away when you have questions or need help making decisions on next steps for your loved one’s care.

    We also provide quarterly follow-ups and help monitor changes in the health and lifestyle of your senior.

  1. Initial Assessment

  2. In-person follow-up one week after initial assessment to present the report and highlight recommendations

  3. Follow up every 3 to 6 months (as requested)

  4. Emergency Assessments for status changes

  5. Advocacy at doctor appointments or family discussions is available upon request
